For specialty clinics

Referred patients arrive with their history already read.

Referred patients bring scans, procedure reports and labs from systems you can’t see into. Triangle requests those outside records, reads them, and gives your team one current history before the first visit.

HIPAA compliant. Each patient signs their own release. Clinical decisions stay with your team.

How it works

From referral to a readable history. Step by step.

The patient consents and Triangle does the requesting and the reading. Your team reads the result.

  1. Step 01

    Set up your clinic

Your clinicians create accounts at app.trianglehealth.com/practitioner. For a program across the clinic, we set it up with you.

  2. Step 02

    Invite the referred patient

At referral, your team invites the patient, or the family member who helps them, to create a Triangle profile. Your clinicians join the patient’s care team.

  3. Step 03

    The patient consents

The patient verifies their identity and signs one release, and a clinician signs the records request with them. The patient decides who can see their profile.

  4. Step 04

    Triangle requests the outside records

Through Carequality and TEFCA, the national networks that connect many hospitals and clinics. The patient can upload anything else: reports, discs, faxes, photos of paper records.

  5. Step 05

Your team reads one history before the visit

A readable summary, prior imaging and procedure reports in order, medications and labs, and what may still be missing, flagged up front.

  6. Step 06

    It stays current

As new records arrive the history updates, so follow-ups and second visits start from the same place.

A pilot you can measure

Three numbers worth tracking from the first referral.

Run it on a set of referrals and compare against how they arrive today.

01

Time to a usable history

From the referral to a summary your team can read before the visit.

02

Outside records retrieved

Documents and sources pulled in for each patient, and what is still missing.

03

Duplicate imaging avoided

Prior scans and procedures found on file before a repeat study is ordered.

What each side gets

Your team gets the history. Your patients get their record.

  • One current history they can read
  • Family who help them can see it too
  • Reminders in plain words
  • They choose who sees their profile

Questions

What clinics ask first.

Which hospitals can Triangle get records from?

Records come through Carequality and TEFCA, the national networks that connect many US hospitals and clinics. Not every provider is reachable through them, so the patient can upload the rest, and Triangle shows what may still be missing.

How long do records take?

It depends on the source. The history fills in as records arrive and updates when new ones come in, so the summary always says what it does and doesn’t cover.

Who consents, and who can see the record?

The patient verifies their identity and signs one release; a clinician signs the request with them. The patient decides who is on their care team, and can remove anyone at any time.

Does Triangle make clinical decisions?

No. Triangle gathers and reads records and helps your team prepare. It does not diagnose, treat or prescribe; clinical decisions stay with you.

What does it cost?

A patient’s own Triangle account starts with a 7-day free trial, then $9 a month. For a clinic program or a pilot, talk to us.
