2:40 pm New patient Referral says HTN, CKD, please evaluate. That’s the whole history.

Your new patient’s outside records, read before they walk in.

Prechart requests a new patient’s outside records through Carequality and TEFCA and gives you a readable summary before the visit, so the appointment starts from the history instead of rebuilding it.

The problem

The first visit starts with a records gap

New patients arrive with a referral letter, a medication list from memory, and a history spread across health systems you can’t see into. The fax request goes out, and the records arrive after the patient has gone home.

Without outside records

  • A referral letter and the patient’s recollection
  • Medications reconstructed in the room
  • Outside records requested by fax, arriving later
  • Visit time spent rebuilding the history

With Prechart

  • Outside records requested through Carequality and TEFCA
  • Medications listed from the records, with pharmacy fills when available
  • A readable summary of the history before the visit
  • What may be missing, shown up front

How it works

From referral to readable history

The patient verifies their identity and signs the release. Triangle does the requesting and the reading; you read the result.

  1. Create your clinician account

    Sign up at app.trianglehealth.com/practitioner/.

  2. The patient consents

The patient verifies their identity and signs the release, and a clinician signs the request with them. The patient decides who can see their profile.

  3. Triangle requests the records

Through Carequality and TEFCA, the national networks that connect many hospitals and clinics. The patient can upload anything else: PDFs, discs, faxes, photos of paper records.

  4. Read it before the visit

One current history with a readable summary, structured labs and medications, and what may be missing shown up front.

What you get

The history, organized for review

Everything below is part of Triangle today.

Outside records, without the fax chase

Requested through the national networks, plus anything the patient uploads: PDFs, photos of paper records, discs and faxes.

The history in one read

A summary of the patient’s medical history in plain language, ready before the visit.

Trends across health systems

Lab results become structured results you can follow over time, whichever system ran them.

Medications you don’t rebuild in the room

Listed from the records, including pharmacy fill data when the records contain it.

Gaps flagged up front

Triangle shows which records may be missing and helps request them, so you know what the summary doesn’t cover.

Patients who share with you

Patients already using Triangle can share their profile with their doctors and choose who can see it.

Running a practice? Triangle also offers dashboards built around the CMS ACCESS program’s kidney and cardiometabolic measures (blood pressure, weight and BMI, HbA1c, LDL, kidney function), and population views across your patients.

Talk to Triangle

AI assistants, theirs and yours

When a patient asks about Claude or ChatGPT

Patients who already use an AI assistant can connect it to their own Triangle profile, so it answers from their records, labs and medications. Here is what to tell them.

Step-by-step guides for ChatGPT, Claude, Grok, Muse and others

You can connect yours too

Once you’re on a patient’s team in Triangle, you can connect your own assistant to that patient’s profile and choose what it can read and change. The patient can connect theirs, and so can a family member who helps manage the profile. Each person sees and disconnects their own connections.

Works with
Claude, ChatGPT on Business, Enterprise or Edu (or Pro, read-only), Grok on a paid plan, and other assistants that support MCP (Model Context Protocol).
Reads
Only what the person connecting it allows: records, labs and measurements, medications, notes, care team, wearable readings, genetic reports and the summary of their history.
Limits
It works only with data inside Triangle. It can’t change anything at a hospital or clinic.
Privacy
What the assistant reads is then handled under that assistant’s own privacy policy, which Triangle does not control. Connect only assistants you trust, and tell patients the same.
Control
Each person can disconnect their own connections any time in Triangle under Settings → AI service connections, and access stops immediately.

Trust

Built for patient records

Prechart is made by Triangle, and accounts are Triangle accounts. These are Triangle’s commitments.

HIPAA compliant

Triangle is HIPAA compliant. Details are in its Trust Center.

Never sold

Patient data is never sold or shared for advertising. Patients decide what to share and with whom.

No outside payers

No drug maker, laboratory or hospital pays Triangle, so nothing in a summary is there to sell anything.

Encrypted

Everything is encrypted, and records are the patient’s to delete. Triangle is listed in the Medicare App Library.

Triangle provides research and educational support. It does not diagnose, treat, or prescribe, and it never replaces your care team. Clinical decisions stay with you.

Questions

What clinicians ask

What does it cost a clinician?

Talk to Triangle about pricing for clinicians and for practice-wide tools, such as the ACCESS dashboards and population views.

Patients’ own accounts start with a 7-day free trial (a card is required; cancel before day 7 and they’re not charged), then $9 a month. That covers Triangle requesting their records and keeping them updated, chat with the assistant that answers from their own records, a review of their medical history by a licensed clinician, connected wearables and uploads.

What does the patient have to do?

Verify their identity and sign the release; a clinician signs the request with them. They can upload anything else they have, and they decide who can see their profile.

Will it reach the systems my patients come from?

Triangle requests records through Carequality and TEFCA, the national networks that connect many hospitals and clinics. Not every provider is reachable that way, so patients can upload the rest, and the summary shows what may be missing. Records are assembled into one current history that updates as new ones arrive.

Does it diagnose or suggest treatment?

No. Triangle does not diagnose, treat or prescribe. It organizes outside records and summarizes them so you start the visit already knowing the history. Clinical decisions stay with you.

Is it HIPAA compliant?

Yes. Triangle is HIPAA compliant, and its Trust Center has the details. It is also listed in the Medicare App Library.

Can my whole practice use it?
