For concierge and direct primary care practices

Know a new member’s history before they arrive

Your members join for a doctor who knows them. Folio requests each new member’s outside records through the national networks and turns them into a readable summary before the first visit, so the hour goes to the member, not the chart.

Each member decides what to share with your practice.

Folio is made by Triangle. You’ll create your clinician account at trianglehealth.com.

  • Requested through Carequality and TEFCA, the national health-record networks
  • HIPAA compliant, with its controls in a public Trust Center
  • No drug maker, laboratory or hospital pays Triangle
Illustrative example

The first visit often starts with a records chase

A new member arrives with years of care from somewhere else. Getting it in front of you takes more work than it should.

Requests go out

Your staff faxes release forms and follows up with each hospital and specialist the member has seen.

Pieces come back

Some arrive as PDFs or paper, and some never arrive. The member has forgotten which portal holds the rest.

You assemble it

The history gets read the night before or rebuilt in the exam room, on time your members are paying for.

How it works

From sign-up to a summary you can read

  1. Create your clinician account

Sign up as a clinician with Triangle. Practice-wide arrangements are a conversation with Triangle after you sign up.

  2. Each new member sets up their profile

They start their own Triangle account, verify their identity and address, sign the consent forms and book an intake visit with a clinician.

  3. Triangle gathers their records

Triangle requests outside records through the national networks, Carequality and TEFCA. Members can upload what they have too: PDFs, photos of paper records, discs and faxes.

  4. They share, and you read

Once a member shares their profile with your practice, read a summary of their history, with lab results over time and medicines from the records.

What you get

Service on both sides of the visit

Your practice gets the history. Your members get a record of their own that keeps up between visits.

Looking across your whole panel? Triangle also offers population views across a practice’s patients. Talk to Triangle about it after you sign up.

What it looks like

The night before a first visit

Outside records, read and summarized: the history, the trend that matters, the medicine that stopped being filled, and the record that hasn’t arrived yet.

For members who use an AI assistant

Their own assistant, reading their own record

Members who use Claude, ChatGPT or Grok can connect it to their Triangle profile. With their permission it can read their records, labs, medications and more, so they can ask about their history in the assistant they already use.

Each person connects their own

You can connect yours too

Once you’re on a member’s team in Triangle, you can connect your own assistant to that member’s profile and choose what it can read and change. The member can connect theirs, and so can a family member who helps manage the profile.

Each person chooses
What their assistant can read or change, with separate Read and Write switches for each category, and which profile to share.
Privacy
What the assistant reads is then handled under that assistant’s own privacy policy, which Triangle does not control. Connect only assistants you trust.
Any time
Each person can disconnect their own connections in Triangle under Settings → AI service connections, and access stops immediately. It works only with data inside Triangle and can’t change anything at a hospital or clinic.

  • “What has changed in my lab results this year?”
  • “When did I last fill my alendronate?”
  • “Help me write down questions for my first visit on Tuesday.”

Questions practices ask

What does it cost?
Talk to Triangle about pricing for your clinician account and any arrangement across your practice. Members’ own accounts start with a 7-day free trial; a card is required, and if they cancel before day 7 they’re not charged. After that it’s $9 a month, covering Triangle requesting their records and keeping them updated, the chat that answers from their own records, a review of their medical history by a licensed clinician, connected wearables and uploading anything else they have. Members can cancel any month, and their records stay theirs.
What does a new member have to do?
Start their Triangle account with the 7-day free trial, say who the profile is for, then verify their identity and address, choose who can see the profile, sign the consent forms and book an intake visit with a clinician. They can upload what they already have, and share their profile with your practice.
Where do the records come from?
Triangle requests them through the national health-record networks, Carequality and TEFCA. Not every provider can be reached that way, so members can also upload PDFs, photos of paper records, discs and faxes. Triangle shows what may be missing and helps request it.
Who controls a member’s record?
The member. They choose who can see their profile and who can help manage it, and they can delete their records. Member data is never sold or shared for advertising. Folio is made by Triangle, and you’ll create your clinician account at trianglehealth.com under Triangle’s privacy policy and terms.
Does Folio make clinical recommendations?
No. Triangle does not diagnose, treat or prescribe, and it never replaces the care team. It gathers records and makes them readable. Clinical decisions stay with you and your members.
