Stipple AI agentsAboutTalk to us
For care management and population health teams

You're responsible for patients whose records you can't see.

Stipple gathers your patients' outside records through the national health-record networks and shows your panel in one population view, so care managers act on the picture instead of assembling it chart by chart.

Talk to us

A conversation first. We'll be straight about what's ready today and what isn't.

Stipple is made by Triangle. Your email goes to Triangle's team. Learn about Triangle at trianglehealth.com.

The problem

The picture arrives in pieces.

Patients see specialists, visit emergency departments and fill prescriptions outside your walls. What comes back arrives as faxes and PDFs, if it arrives at all.

So care managers spend their days assembling the picture before they can act on it, one chart at a time.

How it works

A conversation, consent, collection, then one view.

  1. We talk first

Tell us about your program, your patients and the measures you're working on. We'll say plainly what's ready today and what isn't.

  2. Patients consent, one by one

Each patient sets up their own account: an identity check, their address, who can see their profile, and signed consent forms. A clinician signs the records request with them, and their setup includes an intake visit with a clinician.

  3. Records come in through the networks

Requests go through Carequality and TEFCA, and patients can upload what they already have. Each patient's records become one current history that updates as new records arrive, with gaps shown.

  4. Your team works from one view

Population views across your patients, dashboards around the CMS ACCESS program's measures, and a readable summary of each patient's outside records.

What your team gets

What's ready to talk about today.

Your panel in one view

Population views across your patients, instead of one chart at a time.

Where you stand on ACCESS measures

Built around the CMS ACCESS program's kidney and cardiometabolic measures: blood pressure, weight and BMI, HbA1c, LDL and kidney function.

Fewer faxes to chase

Outside records requested through Carequality and TEFCA and assembled into one current history per patient, with what may be missing shown.

A readable summary before the visit

A new patient's outside records and a plain summary, ready before they arrive.

Which way each patient's labs are moving

Lab results become structured results you can follow for each patient, with plain-language explanations.

Medications, including fills

Medications listed from each patient's records, including pharmacy fill data when the records contain it.

Patients' own assistants

Patients can bring their own record into Claude or ChatGPT.

What a patient might ask it
  • "What did my discharge summary say about my medications?"
  • "When was my last HbA1c, and what was it?"
  • "Remind me every morning at 8 to take my blood pressure."

A patient who uses an AI assistant can connect it to their own Triangle profile. With their permission it can read their records, labs and measurements, medications and the summary of their history, and answer from them.

Step-by-step guides for ChatGPT, Claude, Grok, Muse and others

Your clinicians can connect theirs too. A clinician on a patient's team in Triangle can connect their own assistant to that patient's profile, and so can a family member who helps manage it. Each person chooses what their assistant can see.

  • Other assistants that support MCP (Model Context Protocol) work too.
  • What the assistant reads is then handled under that assistant's own privacy policy, which Triangle does not control. Connect only assistants you trust.
  • It works only with data inside Triangle and can't change anything at a hospital or clinic.
  • Each person can disconnect their own connections any time in Triangle under Settings → AI service connections, and access stops immediately.

Questions

Plain answers.

Is this built for health systems and health plans?

The population views and measure dashboards exist today and were built with practices in mind. If you run care management at a health system or a health plan, tell us about your program and we'll say plainly what fits now and what doesn't.

Will we see every outside record?

Not every one. Records come in through the national health-record networks, Carequality and TEFCA, which don't reach every hospital or clinic. Patients can upload what they have, such as PDFs, photos of paper records and faxes, and each history shows what may be missing so it can be requested.

Does it connect to our EHR?

Records come in through the networks with each patient's signed consent. Ask us about your EHR and we'll tell you what's possible today.

How do patients consent?

Each patient sets up their own account: they verify their identity, confirm their address, choose who can see their profile and sign the consent forms, and a clinician signs the records request with them. Their setup also includes an intake visit with a clinician. Patients can change who sees their profile later.

What does it cost?

Pricing for teams starts with a conversation about what you need.

Patients' own accounts start with a 7-day free trial, with a card, then cost $9 a month. If they cancel before day 7, they're not charged, and they can cancel any month after that. Membership covers Triangle requesting their records and keeping them updated, chat with the assistant that answers from their own records, a review of their medical history by a licensed clinician, connected wearables, and uploading anything else they have.

Does Triangle diagnose or prescribe?

No. Triangle helps clinicians and patients understand records. It does not diagnose, treat or prescribe, and clinical decisions stay with your team.

A short email is enough: your team, your patients, and the measures you're working on.
